Fee: $50.00 per license – The city accepts VISA, Discover, MasterCard, check & cash. Please mail, call or bring your money to City Hall.

Please attach certificate of insurance as per City Code 4-201 and 4-301. ($500,000 each occurrence/ $1,000,000 aggregate), and a copy of a current license issued by another city or proof of passing the Prometric test.
